As a three-time gold medallist for the United States, what would be your advice to Team GB as they prepare for the Olympics? My advice would be to figure out what you do well and do that. You can't try to play a certain style, you have to figure out what you do well and hang your hat on that. If you try to take on the personality of other teams and their style of play, it can get confusing. You have years to prepare for the rigor of the Olympics, but you have to try and do things that make you feel good. Figure out what works for you and go from there.

Are the gold medals the proudest achievement in your career? It's by far my greatest achievement. When you look at the world against your own country, the world is a much bigger scale, and for me I've looked at the Olympics as head and shoulders above what else I've participated in. The WNBA, the NCAA championships, all of it has its place. But at the same time, to represent your country is a much bigger platform.

The WNBA is entering its 15th season. Are you proud of the resiliency the league has shown? Absolutely. That fact it's been in existence for fifteen years is fifteen years of dangling carrots in front of little girls, who can pick up the sport and see they can make a career out of it. They can go to college and instead of going to corporate America or being homemakers, they can have a career, play in their own country and have their dreams realized. Collegiate players now have opportunities to continue their careers that will allow them to play their sport in their prime.

British women are now playing Division I college basketball, with Azania Stewart being a notable example. As a college coach, does this bode well for the future of British women's hoops, and on this trip do you expect to see talent capable of playing in Division I? On this trip, I did see the Amaechi Centre in Manchester, and I did see some younger players - 12 and under - who were putting in the time and working hard on the floor. I did talk to a gentleman there who told me there are some players that we should be looking out for. I'm eager to be checking it out, and I hope he's not telling other coaches! Anytime you host an Olympic Games, you see an influx of young people picking up the game. The players who already have a ball in their hands get better. They are eager to do the same to represent their country, as anytime you expose somebody to a higher level of sport they see an outlet.

Even before a physical examination has been made. Indeed in the early stages abnormal signs in the chest may not be detectable, and the diagnosis will depend upon the history and the general appearance of the patient. Typhoid fever on the other hand starts so insidiously that patient may continue with their normal occupation for a few days before seeking advice.

The following signs are common to most infections.

Pyrexia. Normal body temperature shows a diurnal variation of about 1 C. or a rise above the maximum normal temperatures just mentioned.

The rate of rise and fall of fever may be of diagnostic value. A rigor raises body temperature rapidly in spite of every indication that the patient feels cold. Heat production is increased by shivering and heat loss is decreased by constriction of the skin vessels. Rigors are common in pneumococcal pneumonia, acute pyelonephritis, septicaemia and malaria. Subsidence of pyrexia may be rapid by ‘crisis’ or slow by ‘lysis’. Characteristic types of fever, when they occur, are mentioned under specific conditions.

At the height of the fever the skin is hot, flushed and dry. Sweating occurs as the temperature subsides. In rheumatic fever sweating is prominent throughout. Profuse perspiration may lad to a sweat rash (sudamina) consisting of numerous minute vesicles.

Causes of fever other than infection include exercise, a hot bath, the injection of pyrogens, sensitivity to foods and drugs, severe anaemia, venous thrombosis, pulmonary, myocardial or other infarctions, connective tissue disorders, malignant growth, especially when associated with necrosis or generalized dissemination, Hodgkin’s disease and pontine haemorrhage. An apparent rise in body temperature is produced not frequently in malingerers.

Cardiovascular Signs. The heart rate is increased. There is a slight rise in the systolic and fall in the diastolic blood pressure. The result is a rapid pulse of large amplitude which is often described as bounding. For every 0.5 C. rise in temperature an increase of about 10 beats per minute in the pulse rate may be anticipated. Exceptions to this general rule are acute tuberculosis, scarlet fever and acute rheumatic carditis in which it is faster, and enteric fever, viral infections and often meningitis in which it is slower than expected from the height of the temperature.

Respiratory Signs. The respiratory rate is increased by fever in proportion to the pulse. The normal ratio of 1:4 is maintained except with acute infections of the respiratory tract in which this ratio is increased to 1:3 or 1:2.
